1. Material exceptions

Critical and high events still open, preferably with severity and aging.

2. Dollar exposure

Open value at risk, not simply total transaction volume.

3. Overdue corrective actions

Issues whose owners or due dates have slipped.

4. Red KPIs

Only metrics outside threshold with an explicit next action.

5. Weak controls

Missing, failed, untested or overdue control remediation.

A short queue of leadership choices that unblock progress.

Dashboard rule: if a metric can turn red without creating an owner or next action, it is reporting—not operating control.
